Who Are We?

The Altrix Group is a physician-directed, in-home wound care practice serving the greater Houston area. We specialize in the evaluation and treatment of chronic, complex, and non-healing wounds, delivering advanced clinical care directly to patients in their homes to improve access, comfort, and continuity of treatment.

Our wound care specialists provide evidence-based management for pressure injuries, venous and arterial ulcers, diabetic foot wounds, and complex tissue loss, including advanced dressings, skin substitutes, and regenerative therapies. Every plan of care is individualized, clinically driven, and coordinated with each patient’s referring providers to support safe healing, reduce complications, and improve long-term outcomes. We accept Medicare and work with a wide range of commercial insurance providers to ensure medically necessary wound care remains accessible.

Pressure Injuries

Comprehensive in-home evaluation and treatment of pressure injuries at all stages. Our wound care specialists provide sharp and conservative debridement when indicated, pressure-relief strategies, advanced dressings, and infection prevention to support safe, comfortable healing in your home.

Venous Ulcers

Specialized in-home care for venous leg ulcers caused by chronic venous insufficiency. Treatment focuses on evidence-based compression therapy, edema control, wound bed preparation, and advanced dressings to promote healing and reduce recurrence.

Skin Substitutes

Advanced cellular and tissue-based skin substitute therapies delivered in the home for complex or non-healing wounds. These therapies support tissue regeneration, improve granulation, and help advance wounds that have not responded to standard treatment.

Arterial Ulcers

Targeted in-home management of arterial and ischemic wounds associated with peripheral arterial disease. Care includes vascular risk assessment, protection of fragile tissue, infection control, and coordination with referring and vascular providers when revascularization is required.

Specialized Dressing and Topical Applications

Personalized selection and application of advanced wound dressings and topical therapies based on wound type, depth, drainage, and bioburden. Treatment is designed to maintain optimal moisture balance, control bacteria, and create an ideal environment for healing at home.

PRP Therapy for Diabetic Foot Ulcers 

In-home platelet-rich plasma (PRP) therapy for chronic and high-risk diabetic foot ulcers. PRP uses a patient’s own concentrated growth factors to support tissue repair, stimulate healing, and improve outcomes in wounds that are difficult to treat.

Meet Our Provider

Ida Centineo, FNP-C, CWS

Ida is a board-certified Family Nurse Practitioner (FNP-C) and a Certified Wound Specialist through the American Board of Wound Management, with nearly 30 years of experience in nursing and advanced clinical practice.

She first earned a master’s degree specializing in nursing education, which established a strong foundation in teaching, clinical reasoning, and patient education. She later pursued a second master’s degree to become a Family Nurse Practitioner, allowing her to pair advanced clinical expertise with an educator’s approach to care.

Ida’s clinical background spans multiple areas of medicine, including plastic and reconstructive surgery, where she was first introduced to acute wound care and complex tissue healing. She also worked extensively in ambulatory surgery settings, providing her with a deep understanding of both surgical and non-surgical wound management across the full continuum of care. In addition, she has served as an adjunct college instructor for several institutions across the East Coast.

A graduate of one of New York City’s top nursing programs, Ida remains deeply committed to evidence-based practice, education, and individualized care. Through her mobile wound care practice, she provides advanced, patient-centered wound management directly in the home and community setting, with a strong emphasis on prevention, education, and long-term healing outcomes.
